+ What is a 1099-HC, 1095-A, or 1095-B form?

These forms provide proof that you have maintained the minimum required health coverage over the past tax year (Minimum Credible Coverage). 1099-HC is the form required for Massachusetts, and 1095-A or 1095-B is the form required for the federal government. You may need both a 1099-HC and a 1095-A or 1095-B to complete your taxes.

If you received an Advance Premium Tax Credit in 2016, you must file a federal income tax return with the information from your 1095-A form. If you don’t file, you won’t be able to get this tax credit again in the future.

You must meet Minimum Creditable Coverage to avoid tax penalties. Health insurers must let consumers know if their plans meet these standards.

+ Who receives a 1099-HC form?

Massachusetts residents age 18 and older who had health insurance during 2016 will receive a 1099-HC form in the mail:

  • MassHealth members will receive a 1099-HC form from MassHealth
  • BMC HealthNet Plan ConnectorCare and Qualified Health Plan members will receive a 1099-HC form from us.

+ Who receives a 1095-A or 1095-B form?

All Qualified Health Plan members including ConnectorCare, who had coverage during 2016 will receive a 1095-A form in the mail from the Health Connector (except members covered as part of a small group). Members enrolled in a QHP Business Express Group will receive a 1095-B form directly from BMC HealthNet Plan. You may need the information on this form to file your federal tax return. You must file a federal income tax return with the information from your 1095-A form if you received an Advance Premium Tax Credit in 2016. If you don’t file, you won’t be able to get this tax credit again in the future.

+ What do I do with my 1099-HC and/or 1095-A or 1095-B form?

You will use these forms to prove that you had health insurance during 2016 when you file your taxes. The information on the 1099-HC is reported directly to the Massachusetts Department of Revenue, and the information on the 1095-A or 1095-B is reported to the Internal Revenue System (IRS).

+ What if I was covered by more than one health insurance company in 2016?

You should receive 1099-HC and 1095-A or 1095-B forms from each company you had health insurance coverage with, indicating the months you were covered by them during 2016. You will need all of your forms when you file your 2016 tax return.

+ Why didn’t I receive a 1099-HC form?

You may not have received a 1099-HC form if you are:

  • A dependent of another person in your household. That person is the main health insurance subscriber and will receive one form for all of their dependents.
  • A Medicare member.

+ What do I do if I didn't receive my forms? 

You should receive your forms before you file your taxes. If you file your taxes before you receive your forms, you may need to amend your tax return. If you haven't received your form by February 10, 2017, please use the following contact information to make sure you have the forms you need.
