Clinical Programs

BMC HealthNet Plan partners with providers to develop and implement programs that focus on wellness, health maintenance, prevention, and disease management. Care management resources include condition-specific programs and services for any member with complex medical, behavioral or social needs. Care management programs for members with specific conditions include a personal care manager, a personal care management plan, educational materials, and collaboration with the members’ clinical team. Examples of programs include:
