How do I Sign Up?
STEP 1. Apply to MassHealth
Before you can choose a BMC HealthNet plan – MassHealth or Commonwealth Care –
you must apply to MassHealth. MassHealth determines which state program you are
eligible for.
You must complete a Medical Benefit Request (MBR) form.

Keep in mind that when you apply, you’ll need:

Two recent pay stubs or
-
Last year’s U.S. tax return (if self-employed or have rental income) or
-
Social Security award letter
AND
-
Other income (i.e., child support, unemployment, etc.)

Proof of citizenship/national status
(copy both sides)

U.S. passport or
-
Certificate of Naturalization (DHS# N-550 or N-570) or
-
Certificate of U.S. Citizenship (DHS# N-560 or N-561) or
-
U.S. birth certificate and a Massachusetts ID or driver’s license

Service Center at 800-841-2900 (TTY 800-947-4648) from 8 a.m. to 5 p.m., Monday
through Friday.

Step 2. Once You Qualify, Choose BMC HealthNet Plan
Once you qualify for MassHealth or Commonwealth Care, the state will send
you an enrollment package that tells you how to select a health plan.

If you qualify for MassHealth,
you must call 800-841-2900 to choose BMC HealthNet Plan as your health plan.

If you qualify for Commonwealth Care, you must either call
877-623-6765 to choose BMC HealthNet Plan, or do so online at
www.mahealthconnector.org.
